Job summary
Community Urgent Care Service - Rapid Intervention Team
The post holder will be a Registered Healthcare Professional and hold an MSc in Advanced Clinical Practice . They will be an independent Non-Medical Prescriber. They will practice at an advanced level demonstrating in depth knowledge and competence in all aspects of acute care within the patient's home setting & environment, encompassing enhanced assessment, critical thinking and clinical management skills that are evidence based and deliver high quality patient centered care.The service currently operates 24/ 7 including bank holidays therefore team members will be expected to work shifts on a set rolling rota basis. . As this is a community based post a full UK driving license and access to a car is essential as we do cross Wolverhampton borders to accommodate out of area patient assessments.
Main duties of the job
The successful candidate will be part of a dynamic and flexible service providing unplanned and urgent care. The team support patients through acute episodes of ill health in their own homes as an alternative to hospital. They provide enhanced and complex care in a crisis before transferring stable patients to our Planned Care colleagues. You will join a dedicated multi-disciplinary team, who offer a full range of specialist services in order to facilitate a timely, safe and effective patient's journey throughout the health care system. Adhering to 2 hour response times.Essential qualifications include Advanced Health Assessment and V300 Independent Prescribing, MSc in Advanced Clinical Practice
