Job summary
The Health Inclusion Team (H.I.T) is specialist team working within The Partnering Families Team.
H.I.T works with vulnerable families and children aged 0-19 in Wolverhampton who are at risk of being socially excluded and therefore face barriers when accessing health care resulting in poor health outcomes.
Examples of these groups are Gypsy and Traveler families, refugees and migrants, families fleeing domestic violence and those who are homeless and living in temporary accommodation.
Our aim is build a trusting relationships showing kindness, empathy and a non-judgmental attitude to the individual's circumstance ensuring that we communicate with individuals in a manner that allows them to understand and use information to make decisions about their health (GOV.UK, 2021).
Main duties of the job
To recruit and engage eligible, hard to reach families within the Gypsy, Travelling, Refugee and Migrant communities & movements into temporary accommodation including hostels and havens.
To identify, prioritise, develop and implement programmes of care to meet individual & local needs, taking into account cultural diversity & ensuring prioritisation of work load .
To work with families in a variety of settings including home visits and clinic, providing evidence based advice and support.
To undertake family health assessments and child health surveillance in accordance with the Healthy Child Programme to include adults and children and young people, 0 to 19 years.
To provide relevant information to families, to assist informed choices about their own and their family's health.
To take a lead professional role when required where children and young people with additional needs require an integrated package of support from more than one practitioner/service.
Through supervision ensure that expertise in clinical methods and developed, maintained and documented and that high quality service is delivered safeguarding the child and client at all times.
To work in collaboration with statutory and mainstream services to ensure that families become confident in accessing a range of other universal health and support services.
To signpost families and meet the diverse needs of families for whom English is a second language and ensures access to interpreting services and advocacy.
