Job summary

An exciting opportunity has presented for a forward thinking Physician Associate to join our team. We are keen to hear from candidates who are passionate about improving End of Life Care as this will be the area the successful candidate will focus on. You will be raising awareness and providing education, looking at early identification of end of life in patients and creating personalised management plans. The successful candidate will also be working alongside our Community Virtual Ward Team as well as within General Practice.

The successful candidate is required to have completed the Physician Associate Postgraduate Diploma/Masters from an accredited UK university and will be registered with the Physician Associate Managed Voluntary Register (PAMVR) held by the Faculty of Physician Associates (FPA).

Working as a Physician Associate, you'll be a trained health professional, providing crucial support to doctors and regularly dealing with patients. You'll hold a variety of responsibilities and will typically work in general practitioner (GP) surgeries as part of a medical team.

Main duties of the job

Physician Associates work under the supervision of a doctor as part of a medical team. The successful candidate will have direct contact with patients, take medical histories, carryout physical examinations, update records, make diagnoses, analyse test results, and deliver treatment. The successful candidate will work to a professional code of ethics, providing varied support and flexibility in the role, show a caring attitude towards patients, and will have well-developed communication skills to provide treatment plans.

Job description

Job responsibilities

Provide first point of contact for patients presenting with undifferentiated, undiagnosed problems, utilising quality history taking physical examination, problem-solving and clinical decision making skills to establish a working diagnosis and management plan.

Make professional, autonomous decisions in relation to the presenting problems.

Raising awareness and providing education and early identification of end of life.

Conduct home visits, following adequate training.

Triage incoming pathology results and documents.

Act upon alerts (e.g. QOF, local enhanced services etc.) and reminders. during a consultation

Provide clinical services required, within own competencies.

Undertake Surgeries during extended hours (i.e. some evenings and weekends) in accordance with the access needs of the network.

Respond to medical problems presented by patients, including history taking and examination.

Organise investigation, treatment, and referral, where appropriate.

Discuss with the patient their clinical management plan, modifying according to age and comorbidity.

Be able to justify choice of medication. Able to understand the impact of comorbidities and other medications, poly-pharmacy on agent choice and prognosis.

Provide appropriate preventative health care and advice.

Identify community health needs and participate in the development of patient/family-centred strategies to address them.

Adhere to the prescribing formulary, local and national guidelines.

Record clear and contemporaneous IT based consultation notes to agreed standards.

Collect data for audit purposes.

Engage in appropriate lifelong educational and audit activity - as defined by the Royal College of Physicians Faculty of Physician Associates in order to maintain clinical competence and performance, including 50 hours of annual CPD, good verbal and written communication skills and able to communicate complex and sensitive information to patients, relatives or carer CBD

Participate in annual appraisal survey

Participate in, and support other staff, in ensuring the network meets target

Attend meetings, as required

Provide additional enhanced services to the network that meet the needs of the patient population

Undertake additional training where necessary to provide these enhanced services

Be aware of, and compliant with, all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety, CQC and annual QOF Assessment of the Practice

Meet timescales/deadlines for audits and written returns to ensure that the network meets quality standards and receives the designated funding

Participate in clinical governance activity and contribute to the improvement in quality of health outcomes through audit

Alert other team members to issues of Clinical Governance issues, quality and risk

Participate in Significant Event Analysis reviews
