Job summary
Wolverhampton Community Therapy Service is expanding its successful team to provide an enhanced response to admission avoidance situations, including falls response, and hospital discharge support between 8am and 8pm (7 day working pattern). We are looking for a dynamic band 4 Therapy Technician who has excellent communication skills and a flexible person-centred approach to join our supportive team.
This exciting opportunity includes the option of accessing a range of rotations to support career development.
You will need to have extensive experience of working as a Therapy Assistant including rehabilitation and / or community setting.
Our community team includes Physiotherapists, Occupational Therapists, Rehabilitation Assistants, Senior support workers and Therapy Technicians. The service covers the Community Intermediate Care Team (CICT), Rapid Intervention Therapy (RIT) and Community OT and Physiotherapy.
- RIT therapy enables same day access to therapy to prevent hospital admission and involves joint working with urgent care advanced clinical practitioners, nurses, and paramedics with consultant support.
- CICT provides short term (up to six weeks) rehabilitation and re-enablement programmes to patients in their own homes following hospital discharge, illness or injury.
This role involves working extended hours- 7-day working pattern.
A full UK driving license and access to a vehicle that can be used for work is essential.
Main duties of the job
Complete allocated assessments for patients requiring Occupational Therapy or Physiotherapy interventions. Qualified therapists are available for support and guidance as required.
Undertake home visit in the community providing rehabilitation and therapy interventions e.g. issuing mobility and independent living equipment, provide exercise programmes and practising activities of daily living.
Complete accurate patient records and reports
Complete and follow individual patient treatment plans
Effectively communicate with multi-disciplinary team regarding all aspects of patient care.
Assist qualified therapists to undertake joint assessment and treatment interventions in admission avoidance situations.
Complete observations and take appropriate actions
Liaise with a wide range of other services and professionals
Complete recommendations and refer to other services as required

To manage and prioritise designated caseload, working without direct supervision, however, a qualified therapist would always be available for advice and guidance when necessary.
Take responsibility for the onward referral of patients e.g. to Community agencies and Social Services as necessary and in consultation with the referring agents as required.
To take responsibility for carrying out home visits without the direct supervision of a qualified member of staff.
To be responsible for the timely provision of equipment and to take responsibility for the prescription and provision of equipment in area of specialty as necessary.
To fit equipment and instruct patients in correct use of any equipment prescribed following competency training.
To assist Occupational Therapist/Physiotherapist in providing therapy interventions.
To undertake specific assessment techniques as required with in competencies of specialty e.g. muscle testing, blood pressure monitoring.
To attend and actively participate in the delivery of in service training programmes. Training newly appointed Rehabilitation Assistants and others as required in basic elements of patient care, safety and inducting them into area of work.
Encourage patients and their carers in an active approach to regaining and maintaining functional and personal independence, providing direction and guidance This requires empathy, sensitivity, motivational and interpersonal skills.
Use effective communication tools, verbal and non-verbal to communicate treatment programmes to patients who may have barriers to understanding or communicating, or who do not have English as a first language.
Complete accurate documentation of patient contacts in line with the professional standards of the Chartered Society of Physiotherapy (CSP) / Royal College of Occupational Therapy (RCOT) and service standards.
To collate all activity data relating to area of work.
To liaise with the relevant Occupational Therapist/Physiotherapist as necessary regarding patient care, highlighting any variations to expected outcomes from treatment programmes you deliver
To contribute to the induction of new members of qualified staff. Be an active member of the in-service training programme by attendance at, and participation/presentation in, in-service training programmes, tutorials, individual training sessions, external courses and peer review.
Attend multi-disciplinary team (M.D.T) meetings as the Occupational Therapy / Physiotherapy representative for individual patients.
Liaise appropriately with patients, relatives and carers and with the M.D.T.
Carry out domiciliary visits taking responsibility for assessing and managing risk in a community setting as a lone worker.
