Job summary

The Cancer Navigator will form an integral part of the multidisciplinary team approach in caring for the physical, social and psychological needs of patients diagnosed with cancer and their carers throughout the care treatment pathway and after care support.

The Cancer Navigator will work in close unison with the Clinical Nurse Specialists to ensure the coordination and management of patients at all points of their treatment but in particular the Cancer Navigator will also act as a point of contact for patients who will have been prepared by their clinical team for patient initiated follow up (PIFU) - and will be able to sign post patients to support their self-management

The post holder will work independently within the remit of the role but under the guidance and supervision of the Clinical Nurse Specialists:

Work as a Health/Social Care Assistant to assist with the coordination and management of patients undergoing treatment at The Royal Wolverhampton NHS Trust to support patients both on and off treatment.

Provide information to patients related to treatment/follow-up and signpost to appropriate healthcare providers within remit of role and guidance.

Main duties of the job

We are pleased to be recruiting a 12 month fixed term Macmillan funded Care Navigator role into the oncology department. The successful candidate will be fully integrated within the oncology CNS team and working closely with teams support patients through their cancer journey. The post holder will work with the CNS and through the completions of Holistic Needs Assessments ensure that patients receive the support that they need.

Some of the work will be aimed at helping patients with activities of daily living and facilitating client's engagement with community services in Macmillan support services. In addition, the post holders will act as a key contact and support service users to access different interventions provided within the team.

The post holder will receive training in all areas including level 1 psychological training. The role will be supported by and work closely with the Macmillan Cancer Centre within the organisation.

In addition, the post holder will have a role in supporting the senior nurse in the development and delivery of organisational health and wellbeing events in a community setting and increasing the knowledge of the local population and workforce around cancer.

You will have great communication skills and experience of building relationships to overcome barriers to engaging with services. Be able to work autonomously and as part of a team supporting the senior nurse and clinical team manager to deliver this new service
