Job summary

An exciting opportunity to join the Macmillan Team in a role to expand the referral support service.

The Referral Support Care Navigator will form an integral part of the multidisciplinary team approach in caring for the physical, social and psychological needs of patients going through fast-track referrals (2 week wait), vague systems and screening pathways where there is a suspected cancer.

The navigator time will be split between the Referral Support Service and the Macmillan Cancer Support and Information Centre.

Main duties of the job

The post holder will work independently within the remit of the role but under the guidance and supervision of Cancer Services Support Managers. This will include:

  • Work as a Health/Social Care Assistant to assist with the coordination and management of patients undergoing diagnostic, screening fast track and treatment at The Royal Wolverhampton NHS Trust to support patients both on and off treatment.
  • Provide information to patients related to first appointments, treatment/follow-up and signpost to appropriate healthcare providers within remit of role and guidance.
  • Support the fast-track team to book appointments where service dictates.
  • Provide telephone advice and triage where appropriate, working within defined protocols and guidelines. Document and communicate outcomes as per service specification.
  • Provide level 1/2 psychological support within own knowledge boundaries following appropriate training.
  • Perform Holistic Needs Assessments using designated screening tools as appropriate
  • To participate in collating patient surveys/patient experiences surveys and develop action plans alongside the clinical nurse specialist.
