Job summary

It is an exciting time to be joining the Out of Hospital agenda in this newly created role, to lead the advanced practice remit. This lead ACP role will span across Adult Community and Primary Care Services (including frailty provision within the GP home visiting and Enhanced Health in Care Home team) to provide expert clinical practice, guidance and leadership to a team of staff who are working in advanced practice roles. The role is required to provide clinical support and escalation for both directorates, supervision and assurance of competence and practice, prescribing oversight and Governance, and be integral to service development and continuous quality improvement. The role will ensure that staff are competent and confident to work autonomously and have sound decision making to support admission avoidance and care closer to home. The role will also have an emphasis on research and education. The post holder will also be the Non-Medical Prescribing Lead for Community and Primary Care.

Main duties of the job

Advanced level practice encompasses aspects of education, research and management but is firmly grounded in direct compassionate care provision or clinical work with patients, families and populations. Nurses working at an advanced level will provide expert nursing leadership both clinically and professionally, they will manage a defined group of patients from first assessment through examination, investigation, diagnosis and treatment whilst promoting public health and well-being.

80% role clinical

20% role in education/research/service development.

o provide clinical leadership, expert practice, and advanced knowledge, integrating research evidence into practice and contributing to/leading on research

o be accountable for ensuring safe and effective clinical practice; enhancing the patient's experience; ensuring effective contribution to the delivery of the organisation's objectives; managing and developing the performance of the team and managing a devolved budget ensuring efficient and effective use of resources.

o identify research questions, disseminate research findings and promoting their translation into practice

o teach and support professional colleagues and provide skilled professional leadership.
