Job summary

The Shrewsbury and Telford NHS Trust is recruiting Junior Clinical Fellows to join our established Medicine department. The post would provide 4 monthly bases through a range of general medical specialties including Acute Medicine, Care of the Elderly, Cardiology, Respiratory, General Medicine and Diabetes and Endocrinology. This is a brilliant opportunity to join a friendly and dynamic hospital team with a range of specialty interests and an excellent reputation for providing educational opportunities in a supportive environment. This post is not recognised for training.

There is the opportunity for internal progression to ST3+ (IMT3+ equivalent) if the relevant competencies have been met within a clinical setting.

Main duties of the job

Duties will include:

  • To be part of a Consultant lead clinical team providing specialty and general medical care to medical inpatients.
  • To be part of the on-call team covering the Acute Unselected medical Take supported by the STR and lead by the on call Consultant.
  • To develop enhanced clinical skills, especially in the assessment and management of acutely ill patients.
  • To have excellent team-working skills with the abilities and professional attitude to work well in a multi-professional team.
  • To deliver clinical and quality targets agreed both nationally and locally with clinical teams and commissioners.
  • To follow patients throughout their clinical pathways.
  • Contribution to an out of hours on call rota is core to this role

About us

The Shrewsbury and Telford Hospital NHS Trust (SaTH) is the main provider of acute hospital services for around half a million people in Shropshire, Telford & Wrekin and mid Wales.

Encompassing some of the most picturesque parts of England and Wales, the Trust's catchment stretches from the Cambrian Mountains in the west, to Newport and the fringes of the Black Country in the east. Both hospitals provide a wide range of acute hospital services including accident & emergency, outpatients, diagnostics, inpatient medical care and critical care.

We also provide services such as consultant-led outreach clinics at the Wrekin Community Clinic, Telford, the Robert Jones and Agnes Hunt Orthopaedic Hospital, Gobowen and the Bridgnorth, Ludlow and Whitchurch Community Hospitals.

Job description

Job responsibilities

The Department

The post will be based at either The Royal Shrewsbury Hospital or The Princess Royal Hospital, Telford. There will also be an acute medical on call commitment. The teams include a range of Specialist Trainees, Internal Medicine Trainees (IMT) and Foundation level doctors and are supported by physician associates, and advanced clinical practitioners. The post would suit those doctors who have completed their foundation program or trust doctors with some clinical experience who wish to gain further experience in general medical specialties before entering a training program. It is also suitable for foreign trainees with some experience of clinical medicine who are looking for NHS experience before entering a training program at IMT or FY level. It is ideally suited for someone who enjoys working in a stimulating and friendly environment. There is a formal on call commitment with this post that can be tailored to the successful candidates experience.

Educational Supervision and Training

This post is a service appointment, supported by a designated Consultant Educational Supervisor.

The hospital has a number of very experienced educational supervisors and has very close contact with the local deanery. The successful candidate will be provided with an eportfolio and will have a named educational supervisor who is familiar with the foundation and new internal medicine curriculum and will be able to help the successful appointees to achieve their equivalency certificate if desired. The consultant will work with the post holder regarding completion of competencies and career development with PDPs and regular educational meetings. The local Royal College Tutors will supervise the process.

The successful candidate will also be supported through any required MRCP exams and will have access to our PACES teaching program and local IMT and foundation teaching as well as procedural simulation training. We have a high exam pass marks as well as constantly high JEST and GMC survey scores. We have a very high success rate at getting trust doctors into training programs at STRs and IMT level.

There are also weekly educational grand rounds on both sites and weekly acute medical training sessions.

There is a junior doctor meeting at 8.30am on a Monday morning, on both sites, chaired by a Consultant, to address staffing and junior doctor concerns. Medical management and Staffing leads are represented at these meetings.

Developing a Formal Department in General Internal Medicine

The medical needs of the population are changing. The increasing number of elderly patients as well as the growing number of co-morbidities has led to an increase in the number of patients admitted with complex general medical care needs who dont fit easily into a single specialty.

The Joint Committee of Shropshire and Telford & Wrekin Clinical Commissioning Groups has approved plans for the NHS Future Fit. Funding has been approved to develop Acute and General Medical services and modernise facilities with RSH becoming a centre for Specialist Emergency Care and PRH becoming a dedicated Planned Care site.

The national response to this trend towards an ageing population with complex needs and obesity linked multi-pathology has been for training and services to regress from early specialisation and instead emphasise the importance of internal medicine being an integral part of a physicians core role to manage the evolving patient needs.

In creating Internal Physicians, we would provide a more definitive structure for managing unselected medical inpatients beyond the timeframe of an Acute Medicine physician. This in turn would also release current Speciality Consultants providing base ward care in to delivering a mixture of In Reach, Same Day Emergency care and Outpatient clinics instead. Our model includes in house training to ensuring we focus on developing a more robust strategy for creating a strong future workforce.

We are therefore very excited to be developing a formal Department of General Internal Medicine to better care for this cohort of patients. At present we have one Consultant General Internal Physician supported by several competent locum consultants and aim to recruit substantively to build a thriving department. This is a very exciting time to join the team as Health Education England (HEE) funding has been secured for trainees in general internal medicine who will be training towards either their CCT or CESR in General Medicine (a three-year training programme).

Rota Duties

The post-holder will be expected to contribute to the General Internal Medicine full-shift rota. The rota is compliant with the terms and conditions of the 2016 Junior Doctor Contract and WTD and has been carefully designed to maximise training opportunities for doctors whilst maintaining a safe and efficient service, with an even spread of long days and nights. The post holder will be 1 of the team of doctors contributing to the full shift rota across a variety of medical specialties and is prospectively covered for annual and study leave.

The current work pattern may be subject to change depending on the needs of the service but the below timetable provides an ideal of what to expect.
