Job summary
We are looking for a motivated, innovative and friendly Band 4 Assistant Practitioner to join our dynamic and friendly Stroke Early Supported Discharge (ESD) Team at The Shrewsbury and Telford Hospitals NHS Trust. This post is being offered as full-time.
The successful candidate will need to of had previous Band 3 or above Therapy Support Worker experience for either physiotherapy and/ or occupational therapy/ speech and language therapy teams. Additional specific training can be provided to enable the successful candidate to provide therapies through following protocols to autonomously deliver assessment and rehabilitation to ESD Stroke patient's in an individualised, goal directed manner. The ideal candidate will have experience working with patients with stroke and neurological conditions.
The post holder needs to be flexible, have good time management skills, confident IT skills, and work well in a team as well as independently. A car driver is essential as this is largely a community based role.
We are delighted to be able to offer patients who have had a stroke the opportunity of assessment and treatment over the weekend and Bank Holidays and the post holder will also be required to participate in the 7-day working rotacurrently based on the stroke wards at The Princess Royal Hospital in Telford.
Main duties of the job
As the successful candidate you will have your own case load working as part of the multi-disciplinary team delivering therapy to patients with Occupational Therapy, Physiotherapy and Speech and Language therapy needs following stroke. In doing so you shall work predominantly with patients in their own home helping them and their families manage their physical, social and psychological needs post stroke.

Our ESD service provides therapy to stroke patients across Shropshire, Telford and Wrekin and the team are currently all based together in Shrewsbury adjacent to the main hospital site. Team members include Occupational Therapists, Physiotherapists, Speech and Language Therapists, Assistant Practitioners and admin support.
