The Ealing Rapid response/admission avoidance team want to recruit to new staff: Community Specialist Nurse - Intermediate Care. We are seeking a skilled and compassionate Community Specialist Nurse to join our Rapid Response team, delivering high-quality, patient-centred care across community and inpatient settings. The role requires strong IT literacy, including effective use of office systems, the ability to travel independently across the borough and wider network, and confidence in working autonomously. You will demonstrate strong clinical skills, including the ability to assess patients effectively and identify early signs of deterioration, ensuring timely intervention. The post holder must be physically capable of using a range of clinical and non-clinical equipment and delivering comprehensive care to vulnerable individuals. .
Main duties of the job
The role demands sustained concentration, critical thinking, and the ability to analyse your own practice as well as service-level performance. You must be emotionally resilient, able to manage challenging situations involving patients, families, and colleagues with professionalism, empathy, and integrity. Working in varied and sometimes shared environments, you will provide personal care while upholding dignity and safety at all times.The Intermediate Care team is passionate about training and education and the successful candidate will be offered teaching on a regular basis and will have the opportunity to develop in these roles

- Experience and evidence of teaching and assessing e.g. Preceptorship/Mentor
- Commitment to complete Independent Nurse Prescriber course
- Evidence of recent CPD
- Must have full UK driving license and access to a car
Desirable
- Advanced clinical skills - ability to assess, diagnose and treat as an independent practitioner minimum level 6 study or similar
