Lead Diabetes Specialist Nurse

Job summary

We are looking for a Band 8a Lead Diabetes Specialist Nurse, with Non-Medical Prescribing and recognised diabetes qualifications.

The post holder will provide the strategic vision, expert clinical leadership and management to develop, implement and evaluate the diabetes nursing service in West London Forensic services.

The role will require advanced clinical skills and experience. The post holder must be able to instigate and review diabetes treatment for both type 1 and type 2 diabetes. You must be able to adjust insulin according to protocols and titrate medicines safely and accurately in line with Diabetes guidelines, policies and NMC standards.

You will educate GP's, Practice Nurses, ward doctors, nurses and AHP's in supporting their patients with diabetes and working collaboratively with MDT in weight management and prevention of diabetes. You will demonstrate strong decision-making skills with the ability to motivate other non- specialist practitioners into improving their knowledge in the management of the person with diabetes. You will also provide structured education programmes for people with both Type 1, Type 2 and pre-diabetes.

You will be working in Primary Health care, West London Forensic services, in three different buildings within St-Bernard's Hospital site and will also be required to do ward visits.

The service is still being developed, and you will be integral to suggesting new ideas for service improvement and innovative ways of working.

Main duties of the job

  • Work closely with the multidisciplinary team including consultants, GPs, nurses, allied health professionals, social services and voluntary sector partners.
  • Hold and manage an active caseload.Assess patients and develop, implement and evaluate diabetes care plans.
  • Participate in nurse-led, outreach and community clinics, and carry out home visits when required.
  • Undertake clinical procedures appropriate to the role, including initiating and adjusting insulin and recommended oral diabetes medication in line with national and local guidelines.
  • Provide telephone support, assessment and advice to people with diabetes.
  • Act as a resource for colleagues, teams, patients and families, offering guidance and ongoing support.
  • Support newly diagnosed patients and direct them to appropriate service.
  • Ensure practice aligns with clinical governance requirements.Contribute to the development and implementation of clinical guidelines and protocols for the team and wider services.
