Job summary

Are you looking for an exciting opportunity within an expanding Rough Sleepers Team?

We are seeking a motivated individual with strong managerial experience and a background in supporting adults with mental health needs. This role will help lead and develop the Rough Sleepers Tri-Borough Team, which works with people who are sleeping rough and have complex mental health needs. We are also looking to broaden our work to those at risk of homelessness and open to other mental health services.

You will need well-established leadership skills, experience supervising staff in multi-disciplinary teams, strong communication abilities, and confidence in developing partnerships across services. A solid clinical background, trauma-informed practice, and experience engaging with community services and Local Authorities are essential. The role also involves reviewing data and dashboards to monitor referrals and support quality improvement.

The post is based at the Ealing Acton MINT building, with travel expected across H&F, Hounslow, and Ealing. We are looking for someone flexible, creative, and committed to collaborative working, with a good understanding of adult safeguarding and a passion for helping people reintegrate into the community.

The team includes a Consultant Psychiatrist, Social Workers (AMHPs), a Psychologist, an Occupational Therapist, and Community Psychiatric Nurses, working closely through MDTs, CIG, and reflective practice.

Main duties of the job

Work effectively within a multidisciplinary setting, holding self and colleagues to account appropriately and demonstrating personal commitment to integrated (managerial/professional) leadership.

Achieve an agreed set of health performance targets.

Oversee the performance of the team and productivity.

Contribute in the planning and delivery of service objectives, managing specific change projects as required, ensuring customer focus, consistency, quality, and cost effectiveness.

Respond to Serious Untoward Incidents (SUIs), complaints and communications, ensuring timely and thorough investigation, effective communication of recommendations and closely monitoring actions to ensure they are fully implemented

Identify risk and take corrective action in line with trusts integrated governance framework.

Promote and display via personal actions support for the Trust's key policies on Equality and Diversity.

Be able to deputise in the absence of the Team Manager.
