Working Hours: Monday to Friday, 9am to 5pm, closed Bank Holidays.

An exciting opportunity has become available for a permanent Community Mental Health Practitioner to join the Ealing Acton MINT team. You will work as part of a multidisciplinary team while also having the autonomy to manage a caseload of patients with a range of mental health conditions.

The role involves close working with community partners, including GP practices, to ensure the delivery of high quality, community based care. This post plays a key role in breaking down barriers between mental health and physical health services, social care, voluntary and community sector organisations, and primary and secondary care. The aim is to deliver integrated, personalised, place based and well coordinated care for adults and older adults.

Main duties of the job

We are looking for a Community Mental Health Nurse to support people with mental health needs to access the right care at the right time, and to help them navigate services smoothly. The role focuses on empowering individuals to manage their mental health challenges and work towards personalised recovery goals on their own terms, with the support of their families, carers and social networks, and within their local community.

We are seeking applications from Registered Mental Health Nurses who are passionate about delivering high-quality, person-centred care.

The ideal candidate will have:

  • Experience of working with individuals across a range of mental health diagnoses and needs

  • Current registration with the relevant professional body, such as the Nursing and Midwifery Council, Social Work England or the Health and Care Professions Council

  • Knowledge of, and experience in, delivering a range of brief interventions for people with mental health needs

  • Strong written and verbal communication skills, with the ability to build and maintain effective working relationships with local MINT teams and partner services

  • Educated to degree or Diploma level with other courses relevant to primary care mental health services e.g. CBT, Brief Solution Focussed Therapy etc.
  • RGN/physical health qualification
  • o Evidence of recent and on-going relevant further professional development

Experience

  • Demonstrable experience of post qualifying work in psychiatry.
  • o Experience of supervising junior staff within own and/or other disciplines

Desirable

  • Evidence of experience in a Band 6 position or equivalent
