An exciting opportunity has arisen for a suitably qualified and experienced Senior Pathway Coordinator to work closely with the Service Manager, Team Manager and other Senior Clinicians within H&F Central, North & South MINT services in Hammersmith & Fulham. This post is open to all Health Care Professionals, Nurses, Social Workers, Occupational Therapists and is a management post within the team.
Remodelled community teams have been established over the past few years following a successful bid for additional resources. In line with the population health model the team now deliver mental health interventions in partnership with Children and Adolescent Mental Health, primary care and other voluntary sector colleagues as well as continuing to provide care for those with more complex needs focusing on care pathways or transitions of young people aged 16-25 years in adult services.
Background;
The Long-Term Plan, together with NHSE's Community Mental Health Framework, provides a historic opportunity to address gaps in the provision of Mental Health care within communities. Community teams have moved away from siloed, hard-to-reach services towards joined-up care and whole population management approaches, establishing a revitalised purpose.
One of the LTP's key objectives is to develop "new and integrated models of primary and community mental health care [which] will support adults and older adults with severe mental illnesses".
Main duties of the job
The post holder will lead and monitor the performance and care delivery of the Link Worker and/or other professional staff in theMINT team, ensuring local professional leadership of the young adult care pathway. They will participate in review of clinical policies and guidelines relevant to the area and support and implement changes to practice for complex 16 - 25uears olds who may be finding it challenging to engage. Additionally, they will liaise with other partners, stakeholders, organisations and professionals to promote communication and good practice; be able to undertake audit activity and supervision to other staff in service area/care group. They would need to understand psychosocial interventions to include psycho-education, wellness recovery action plans and discharge planning and be knowledgeable in how to access local community resources.
The post holder will also be responsible for the discussions and assessment of care needs and the development, implementation and evaluation of programmes of care for a small and defined caseload; and maintaining associated records.
In addition, the post holder will be responsible for supporting the team managers and clinical staffs in providing effective care planning, leadership within a clinical team providing a high-quality service within the resources available within a defined geographical area whilst integrating a wide range of community activities.
