Job summary

Intermediate care is a service that helps people during a period of severe or sudden illness, or when they have been discharged from a general hospital, so they can be cared for and recover in their own homes or as an interim in Clayponds Hospital.

  1. To provide evidenced based clinical care to patients whilst they recover from a period of poor health or a functional decline and leadership to the MDT.
  2. To contribute to and ensure a safe environment for patients, staff and visitors.
  3. To ensure the successful implementation of care as prescribed by the MDT or yourself.

Main duties of the job

  1. To access, plan and implement care to patients in line with MDT
  2. Undertake advanced clinical assessments and clerk patients on admission.
  3. To create an environment conducive to learning and 'no blame' culture and communicate effectively with all healthcare colleagues, patients and carers.
  4. To ensure that documentation meets the NMC standards for record keeping and confidentiality.
  5. To ensure that an individual programme of care is planned implemented and evaluated.
  6. Work with senior staff to develop and implement care pathways, clinical practice, guidelines and protocol in line with NICE guidance and evidence to deliver quality care.
  7. To be accountable for own professional action adhering to The Nursing and Midwifery Code of Conduct.
  8. To be readily accessible to each service user.
  9. To report to the Ward Manager/Matron and work in line with Trust Policies and protocols.

  • oAdvanced clinical assessment skills at minimum level 6 study or similar course or commitment to complete.
  • oEvidence of recent CPD

Desirable

  • oAdvanced clinical skills - ability to assess, diagnose and treat as an independent practitioner minimum level 6 study or similar

Experience
