Job summary

The post holder will be responsible for delivering a high-quality mental health service to patients under the care of the Crisis Assessment and Home Treatment Team (CAHTT) within secondary mental health services.CAHTT is dedicated to providing timely, flexible, and person-centered interventions aimed at preventing hospital admissions and readmissions. The team focuses on supporting individuals within their communities, tailoring care to meet their unique needs.

The role involves actively contributing to the continuous development and improvement of services, ensuring that effective care pathways and individualised care plans are in place to facilitate urgent and crisis care access. The post holder will champion evidence-based practices by offering clinical advice and support, serving as a role model, and delivering education and training as appropriate.

Close collaboration with carers, statutory and non-statutory services, emergency responders, and third-sector organisations is essential. The post holder will also work closely with local Single Point of Access services to provide rapid responses to mental health crises, helping to reduce unnecessary A&E attendances.

Main duties of the job

To contribute to planning, delivery and monitoring interventions and/or treatmentsTo respect individuals' dignity, wishes and beliefs and obtain their consent for the assessment to be undertaken.To identify appropriate methods, techniques and equipment for different activities taking into account any risks.To undertake reviews/interventions as agreed with the care team, following established protocols/procedures and consistent with legislation, policies and procedures.To report findings in the appropriate format to the people who need them.

To offer to the team his/her own insights into the health and well-being needs and wishes of the service users and make suggestions on the treatment that might be needed.

To maintain awareness of Trust medicine management policy and protocols and ensure adhered to practice guidance, as they are updated and respond to national and local guidance (nursing)

Experience

  • Post qualification or placement experience to include appropriate experience within an acute mental health care setting and / or experience within community, crisis care, or liaison psychiatry
  • Experience of working with other professions within a multi-disciplinary team
  • Experience of working independently
  • To understand and have experience of risk management
  • Experience of receiving supervision

Desirable

  • Experience with telephone triage within a Single Point of Access Service
