This is an exciting opportunity to make a real difference to patients living with long-term respiratory and cardiac conditions, as well as those recovering from Post-COVID Syndrome. You will be part of a specialist community team that delivers high-quality rehabilitation and self-management programmes across Hounslow, helping patients to manage breathlessness, fatigue, reduced mobility, and other symptoms that impact daily life.

You will also assist with home environment assessments, demonstrate safe use of mobility and rehabilitation equipment, and educate patients and carers on breathing techniques, pacing strategies, and energy conservation.
