Job summary
To support the Ealing Bladder and Bowel Health Service in the provision of high quality continence care to patients in their own homes and other clinical settings across Ealing.
The Health Care Assistant works under the supervision of the Continence Specialist Nurses. The post holder will work in collaboration with colleagues across the Trust and liaise with other partnership agencies.

Main duties of the job
1. To carry out routine continence reviews. This would include updating medication list and recent medical interventions, testing urine, bladder scanning, checking pressure areas.
2. To have knowledge of the home delivery service and the products available via the NHS.
3. To have knowledge of how to order commodes or other devices via prescription such as sheaths or urinals,
4. To carry out continence reviews for patients requiring a yearly review or at the request of patient or referring body due to a reported change in a patients requirement.
5. To escalate or raise concerns re patient condition,to monitor and evaluate patient's progress and report any changes in the patient's condition to Continence Nurse or Specialist Nurses.
6. To liaise and communicate with other members of the Primary Health Care Team and other agencies such as home care to provide effective care.
