Job summary

The Hounslow Health Visiting Service is seeking a Band 5 Community Staff Nurse to support the delivery of the Healthy Child Programme. The successful applicant will work within a skill mix team to support and assist in the development, implementation and evaluation of activities aimed at promoting the health of the local population, as well as offering support to identified families as delegated by a Health Visitor. The post is suitable for those wanting to develop the skills and knowledge to prepare for a Health Visiting career

Main duties of the job

The Community Staff Nurse will work in a skill mix Health Visiting team supporting the delivery of the Healthy Child Programme, including

  • Health Review contacts
  • New birth reviews
  • Maternal mood assessment and listening visits.
  • Transfer in Contacts
  • Individual, group and local public health activities
  • Follow up on DNA's
  • Delegated Home visits
  • Infant feeding support
  • Accident prevention

o To support Duty Health Visitor role

o To hold a small caseload of families to deliver delegated packages of care e.g. LAC children, children with SEND needs, premature infants , children with long-term healthcare conditions, listening visits. To report back to named HV and seek regular caseload supervision with the named HV.
