This is an exciting opportunity to join the Early Intervention for Psychosis (EIP) team in the London Borough of Ealing.
We are looking for a highly motivated individual with a proven experience of working in a community based setting. You will be responsible for the delivery of care to an allocated caseload of service users, experiencing, or recovering from first-episode psychosis.
You will provide direct support to service users and carers, guided by the principles of Early Intervention Services. Support which is collaborative, holistic and recovery focused.
You will work as part of a multi-disciplinary Team, working in partnership with a range of stakeholders inside and outside of the Trust.
Main duties of the job
The post holder will be required:
1. To work within the multi-disciplinary team structure of the Early Intervention in Psychosis Team (EIS).
2. To support and work alongside Care Co-ordinators and other team members to assess, plan, implement and evaluate individual programmes of care.
3. To provide Support, give Time to an allocated group of service users, promoting Recovery and maintaining them in the community environment.
4. To make a valuable contribution to multi-disciplinary team assessments and the formulation and implementation of MDT care plans.
5. To maintain a high standard of care, ensuring that the quality of care provided is continually informed by the best evidence-based practice.
Key Result Areas & Performance:
1. To work in close liaison with the other members of the multi-disciplinary EIS.
2. To maximise direct contact with EIS service users and their families/friends and through liaison with other statutory and non-statutory services
3. The central requirement is that the STaR worker will have a willingness to work in a flexible manner and to take on tasks not usually in the remit of any particular discipline. These tasks will be determined by the needs of the individual patient/client e.g. helping with budgeting, shopping, support to access social activities, confidence building.
