Advanced Clinical Practitioner Nurse

Job summary

As an Advanced Clinical Practitioner Nurse you will be one of the senior clinical members of the PCN team who will role model the values of West London NHS Trust, demonstrating a person-centred approach to service delivery and development. You will work collaboratively with the Clinical directors, the PCN staff and wider multidisciplinary team to improve access to primary care for all patients and to support new models of care.

You will use your advanced clinical skills to work independently and your expertise and decision making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated presentations, devising appropriate treatment plans for patients. This will include Initiating, evaluating and modifying a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.

To enjoy this role, you'll need to be a confident and adaptable clinician, self-directed and willing to take on the exciting challenge of working autonomously in the PCN.

Main duties of the job

You will use your four pillars of Advanced Practice to guide your practice and to use your role effectively in the PCN. Whilst the main domain which will be used in this role will be your advanced clinical skills, you will also be able to use the other pillars and provide team leadership, resilience, and determination, managing situations that are unfamiliar, complex or unpredictable and seeking to build confidence in others.

You will take a lead role in the teaching programmes for nurses across primary care.You will also engage in self-directed learning, critically reflecting to maximise clinical skills and knowledge, as well as own potential to lead and develop both care and services.You will also engage in research activity as required, adhering to good research practice guidance, so that evidence-based strategies are developed and applied to enhance quality, safety, productivity, and value for money.
