Job summary
An exciting opportunity has arisen within the Ealing Community Team for People with Learning Disabilities within the Psychiatry Team. The CTPLD is looking for a Band 7 RMN or RNLD to join the team, to support the assessment and case management of people with a Learning Disability with a diagnosed mental health condition. This post is a fixed term contact of one year to cover maternity leave, commencing in March 2024.
The successful candidate will be responsible for implementing the nursing model to assess, plan implement and evaluate care, ensuring people's mental and physical health needs are met; to support hospital avoidance and high quality community care.
You will work alongside the multi-disciplinary team including psychiatry, nursing, psychology, PBS, speech and language therapy, occupational therapy, physiotherapy to ensure a holistic approach is taken to manage health needs.
The successful candidate will work closely with external colleagues; social services, hospital liaison, acute mental health services, AMPHs, commissioners, and other stakeholders.

Main duties of the job

Partnership working with the MDT:

Essential
- Registered Nurse with current NMC Registration. Either RNLD or RMN.
- Educated to degree level or equivalent.
- Advanced clinical assessment skills
- Evidence of training and experience of the application the Mental Capacity Act, Deprivation of Liberty Safeguards and the Mental Health Act.
Desirable
- Further education relating to the physical and mental health needs of people with a learning disability.
Knowledge and Experience
Essential
- Experience of working with people with a learning disability and mental health needs
- Significant post registration experience (excluding post basic training time) with a minimum of 2 years' experience at band 6 and above working in a community service.
- Experience in writing nursing assessments and care plans.
- Advanced clinical skills, ability to assess, diagnose and treat as an independent practitioner or commitment to develop such skills.
- Experience of working independently, autonomously and providing high quality evidence based care.
- Knowledge of NMC Professional Code of Practice and requirements for professional development.
