Job summary

We're looking for some really special peopleto come and join our Ealing Community Partners health and care teams. You need to be caring and positive, hard-working and a good communicator.An interest in the care and rehabilitation of older people is essential.We will provide training and development as needed to the right people. You might be interested in our career progression into nursing and therapy roles or just be looking for somewhere to develop and share your caring skills. Previous experience is not essential, but initiative and a dedication to providing high quality health care is!

Main duties of the job

  • To work as a Nursing and Rehabilitation support workerwithin the Intermediate Care team delivering care to people in the inpatient setting and occasionally in the community as required. There may be a requirement to provide cover in other parts of Ealing Community Partnership to meet the needs of the service.
  • To undertake assessments for a defined and delegated group of patients including near patient observations.
  • To use clinical reasoning/judgement as appropriate when following care plans as directed within protocols and scope of practice.
  • To assist the Intermediate Care nurses, therapists and social workers by carrying out delegated aspects of patient care

  • providing personal assistance to patients with their activity of daily living e.g washing and dressing/ shower, skin care, toileting

  • handling patients with mobility problems, which may include the use of hoist, sliding sheet, wheelchair, mobility aid etc.

  • Experience of working with people who have had a fall
  • Experience of participating in Audit
  • Previous experience in NHS, Healthcare or Social Care
  • Knowledge of aids and equipment used to promote independence
