Main duties of the job
- To work as a Nursing and Rehabilitation support workerwithin the Intermediate Care team delivering care to people in the inpatient setting and occasionally in the community as required. There may be a requirement to provide cover in other parts of Ealing Community Partnership to meet the needs of the service.
- To undertake assessments for a defined and delegated group of patients including near patient observations.
- To use clinical reasoning/judgement as appropriate when following care plans as directed within protocols and scope of practice.
- To assist the Intermediate Care nurses, therapists and social workers by carrying out delegated aspects of patient care

- working as part of a multidisciplinary team with nurses, doctors, pharmacists, therapists etc
- providing personal assistance to patients with their activity of daily living e.g washing and dressing/ shower, skin care, toileting
- undertaking physical measurement monitoring as trained and directed (e.g blood pressure, oxygen saturation, heart rate, ECG etc) and updating care plans accordingly.
- handling patients with mobility problems, which may include the use of hoist, sliding sheet, wheelchair, mobility aid etc.
- making observations, recording in care plans and sharing information with colleagues
