Job summary
We are looking for a band 6 registered nurse with a special interest in diabetes with drive, enthusiasm and vision.This is a developmental role based on completion of required competency.
As a Community Diabetes Specialist Nurse you will play a key role in delivering Community Diabetes Specialist Services to patients. You will also enable and educate GP's, Practice Nurses and Community Health professionals in supporting their patients with diabetes in their usual care setting. The community diabetes service also provides structured education for people with both Type 1 and Type diabetes.
You will have excellent interpersonal and communication skills and will be expected to ensure productive working relationships across acute, community and social care. The role will enable you to develop leadership experience as well as advanced clinical skills. You will be working in the London borough of Ealing in different locations and will also be required to do home visits.
The service is still being developed and you will be integral to suggesting new ideas for service improvement and innovative ways of working.
As a Nurse with active NMC registration and a recognised diabetes qualification you will demonstrate strong decision making skills with the ability to motivate other non-specialist practitioners into improving their knowledge in the management of the person with diabetes.
Main duties of the job
Clinical skills
- Basing own practice on evidence of good practice in diabetes and nursing care, leading evidence based improvements and enabling others to apply them in practice.
- To work closely with the Integrated Diabetes Service and other members of the multidisciplinary team including lead consultants, GP's, nursing colleagues, other health care professionals, social services and voluntary sector services.
- To hold and manage an active caseload.
- To undertake patient assessment, develop, implement and evaluate plans of care for patients with diabetes
- To participate in nurse led clinics, outreach and community clinics and to undertake home visits as required
- To contribute and support the education strategy as agreed with the designated work stream facilitator and Nurse Consultant/Team Leader
- Undertake clinical procedures and interventions commensurate with role, including initiating and adjusting insulin doses as indicated and recommended oral diabetic medication in accordance with national and local guidelines/practice
- Provide telephone support assessment and advice to people with diabetes on their management in accordance with national and local guidelines/practice
