Clinical pharmacist for intermediate care services

Band 7 or 6-7 progression at Ealing community partners (ECP)

An exciting opportunity has arisen for an enthusiastic pharmacist to join a friendly, progressive, clinical team serving our community in Ealing.

We provide medicines management services for in-patients in a 70 bedded community hospital, as well as supporting patients with medicines management in their own homes via our virtual wards services. We deliver specialist rehabilitation, focus on admissions avoidance, support step down patient care and hospital at home for frailty pathways. We aim to provide a calm and welcoming environment.

Medicines management also support our community teams, which include: caring for people with long term conditions, nursing and therapy for children who need specialist healthcare, healthcare for people with a learning disability, physiotherapy, podiatry, SALT and occupational therapy.

The successful candidate should be a highly motivated team player with good communication skills and a keen interest in care of the elderly, treatment of long term conditions and medicines optimisation. Experience of working on medical wards is preferred.

Main duties of the job

  • Providing a clinical pharmacy service to in-patient wards, offering highly specialist pharmaceutical advice to medical staff, nurses and patients on all aspects of safe and effective use of medicines.
  • Providing a clinical pharmacy service to Virtual wards, which supports patients to stay well at home, avoid hospital admission and return home quickly after being discharged from secondary care. In addition to perform deep dive structured medication reviews (SMRs) and domiciliary visits to patients.
  • Supporting our numerous community health speciality centres and clinics including MSK, paediatrics, people with Learning Disabilities, SALT, and inpatients with the delivery of medicines related education and training, medicines information and service improvement using quality improvement methodology

Band 6-7 progression:

You will join us as band 7 if already completed key band 6 objectives, or as band 6, progressing through our training programme to a Band 7.

This post provides the opportunity to develop your clinical skills and knowledge. You will be supported through the Royal Pharmaceutical Society's Foundation Pharmacy Framework to develop a portfolio of evidence

If starting at Band 6, the post holder will move to a band 7 pharmacist role JD / PS, upon achievement of postgraduate qualification such as diploma in clinical pharmacy part 1 or equivalent or IP plus other set band 6 objectives.
