Job summary
The post holder will be responsible for promoting clinical and cost effective wound care management and advice for patients with complex wounds and pressure care management, ensuring a highly responsive and efficient service.
They will be an expert resource for community staff and community hospital staff, ensuring the delivery of evidence based, clinically effective practice to patients They will utilise their highly specialist knowledge and skills to identify priorities and support the development of the tissue viability service.
The post holder will consistently promote best practice. The post holder will support the Tissue Viability Specialist Lead to ensure the delivery of evidence-based care in relation to all aspects of Tissue Viability including leg ulcer management across the service line.
The post holder will improve and maintain standards of patient care through the implementation and continuous evaluation of quality improvement initiatives
Main duties of the job
- To provide professional, clinical expertise in the specialty of tissue viability and act as a resource both in the Community and community hospital settings, including Nursing and residential homes.
- Provide clinical care, ensuring safe practice is delivered within Trust Policy and Procedures and the NMC Professional Code of Conduct
- Provide Complex Wound Care and Leg Ulcer Clinics locally.
- Support the Lead Nurse in practice and/or service improvements taking account of relevant research, clinical guidelines and policy.
- To ensure people with complex tissue viability needs are treated based on evidence-based practice and nationally agreed standards of practice.
- To manage and triage referrals for patients with tissue damage to the specialist service.
- To undertake advanced assessments on patients with complex tissue viability needs using clinical examination skills, detailed history taking and evaluation skills.
- Plan, implement and evaluate individual plans of care utilising advanced knowledge and skills in tissue viability to improve clinical outcomes and escalate as appropriate.
- Complete incident reporting as required and identify learning.
- Offer specialist advice on all aspects of wound care, providing expert clinical guidance and support to health care professionals to meet the standards and protocols required to deliver safe care.
- Advise on the efficient and cost effective use of wound care products, equipment / products and other supplies.
