Job summary

Substantive Consultant Psychiatrist - with dual CCT in Adolescent Forensic psychiatry or single CCT / CESR in Forensic or Child and Adolescent psychiatry with experience of the other specialty.

Part-time applicants also welcome.

Applications are invited from candidates with experience of Adolescent Forensic Psychiatry for a Consultant Forensic CAMHS Psychiatrist post in the North West London Community Forensic CAMHS Team, West London NHS Trust. Although it is preferred that applicants have Higher Training in both Adolescent and Forensic psychiatry, this is not essential if experience in forensic and CAMHS psychiatry can be demonstrated. Applicants must be registered with a licence to practice with the GMC. Inclusion on the Specialist Register or within six months of interview is a requirement. All applicants should have Section 12 Approval and Approved Clinician Status or be able to apply.

Main duties of the job

The vacancy has arisen as the result of the current consultant moving out of London. The service covers 8 boroughs of North West London and is mainly a consultation services. The post holder will be responsible for clinical work, both consultation and direct work, though any consultation recommendations are noted to be based upon multiagency discussion rather than direct assessment.

While primarily responsible for delivering a quality clinical service, the consultant psychiatrist is also expected to be actively involved in the strategic development of the team and broader services, being involved with the team manager and other services in helping to steer the development of the service in line with the strategic direction of the organisation and local Children's agencies.

Job responsibilities

The team receive, on average, 10-12 referrals per month. They work a duty rota to provide a prompt response to each referral and the clinician who initially responds to the referral remains the case holder unless there is a reason to transfer to another team member. Caseloads for each professional are around 12-20 cases, some of which are for consultation and others for more direct work. In additional a team member is allocated as the liaison FCAMHS worker for each borough and attends relevant borough monthly meetings which include the Dynamic Risk Register, the adolescent at risk of exploitation meeting, the transition planning meeting and an offer of monthly consultation to the local CAMHS team. They will also attend the 3monthly YJLD review meeting. Most of these meetings are virtual but when in person they are held in the local service offices.

The team host a monthly FCAMHS case discussion forum to which professionals from Childrens agencies across all 8 Boroughs are welcome to attend.

  1. Clinical responsibilities
  2. The post holder will provide clinical leadership to the FCAMHS and have responsibility to provide / input into the care, treatment and risk management for young people referred to the FCAMHS team. This will include co-working alongside professionals working in other childrens agencies.
  3. The post holder will be on the FCAMHS duty rota and take referrals along with other members of the team. They will have a caseload of between 12-20 cases. These will mainly be consultation cases but there will be some needing full direct assessment or joint intervention. Referrals are received from any local professional working with children and young people and also from secure institutions or psychiatric wards, asking for support in managing risk behaviours. Consultation summaries are prepared after the first consultation. Where needed, more formal multiagency formulations, notes reviews and / or direct assessments will be completed as agreed in discussion with the FCAMHS MDT.
  4. The post holder will support other members of the FCAMHS MDT with their cases, particularly those needing a psychiatric perspective.
  5. The post holder will offer monthly supervision to the West London NHS Trust YJLD workers
  6. The post holder will be the named FCAMHS clinician to liaise with 2-3 local Boroughs, to attend their DSR, young person at risk meeting and offer monthly consultation to local CAMHS.
  7. Since Covid most work has been on Teams but direct work and some meetings are in person at the local referrers base. The weekly Tuesday FCAMHS MDT meeting is in person at St Bernards.
  8. The post holder will offer reciprocal annual and study leave cover for other consultants working in other community forensic services and forensic services.
  9. The post holder will be asked to support the FCAMHS Team with cover if there are urgent clinical needs due staff sickness or unforeseen circumstances.
  10. The post holder will participate in the consultant on-call rota for West London Forensic Services currently approximately 1:20. This on call rota covers the speciality of forensic mental health. On call duties include attendance at seclusion reviews in line with Trust policy and giving telephone advice to colleagues out of hours and if needed to cover an absent consultant. The Consultant rota is supported by junior doctor rotas.

Managerial responsibilities

  1. In addition to providing effective clinical leadership to the MDT, the post holder will have a dual responsibility to contribute to service development.
  2. The post holder will be expected to provide assurance that clinical care is delivered in accordance with Trust policy and processes through normal governance processes in the team and directorate level.
  3. The post holder will be proactively involved in all aspects of clinical governance. These include quality improvement activities that serve the strategic objectives of the Trust, as agreed in the job planning process, through audit and other processes. The expectations are that the post holder will participate in audit activity as required for their revalidation cycle with the GMC.
  4. The post holder will be expected to attend and engage in key governance meetings such as the team and directorate Clinical Improvement Groups.
  5. The post holder will be expected to attend and contribute to the management meetings of the West London Forensic Services and MAC (medical advisory committee).
