Hounslow Mental Health Integrated Network Team (MINT) is a community based service supporting people's mental and physical health as well as social care needs. We work closely with the local GP practices, primary care networks, social services, voluntary sector and other organisations to provide interventions in a more collaborative way which are simpler and support joined-up, personalised treatment and care.
An exciting opportunity has arisen for a senior nurse practitioner with a keen interest in providing outstanding support within mental health community settings. The Community Mental Health Framework represents a new approach to providing community-based integrated support, care and treatment for adults with mental health problems whatever their level of need.

Main duties of the job
Working as a senior member of the Hounslow Mental Health Integrated Network Team you will contribute to the running of the locality team under the direction of the Team and Service Managers, representing the service with external stakeholders. You will be connected with peers across West London NHS Trust and the wider MINT service in Ealing and Hammersmith & Fulham.
Your duties will include carrying a clinical caseload with a complexity commensurate with the banding. You will support duty, triage and allocation of referrals as part of a multi-disciplinary team. With your senior colleagues, your responsibilities will include supervising a team of health and social care professionals and students. You will have delegated responsibility for roster management, incident/ complaint investigations, as well as performance. You will take an active role in recruitment including induction programmes. Through excellent interpersonal skills you will bring a positive outlook to the team.

The postholder will lead and monitor the performance and care delivery of the nursing staff in the MINT team, ensuring local clinical and professional leadership. They will participate in review of clinical policies and guidelines relevant to the area and support and implement changes to practice. The post holder will also be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care for a small and defined caseload; and maintaining associated records.
They will act as liaison with other organisations and professionals to promote communication and good practice; be able to undertake audit activity and supervision to other staff in service area/care group. - Registered Nurse, (mental health) appropriate to the job role.
- Evidence of post registration training in relevant service area
- Completion of Mentorship Course/ENB equivalent/Practice Educators
- Attained at least Level 2 Maths and English (equivalent to GCSE grade 9-4 (A*-C))
- Completion of Mentorship Course/ENB equivalent/Practice Educators
Desirable
- Leadership qualification such as the Edward Jenner programme or similar
Experience
Essential
- Experience of supervising staff and students
- Able to manage a caseload of people with complex mental health needs
- Medication management: experience in intra muscular routes of administration of medicines including depot and vaccinations
