Job summary

POST SUMMARY

The post holder will serve as the Governance and Risk Manager for Wood Green CDC. They will work closely with the WG CDC management team and the Quality and Accreditation Manager to develop, implement, and monitor an effective and reliable governance framework aligned with national best practice.

They will oversee all activities related to patient safety, risk, compliance, and audit, acting as the lead and central point of contact for all CDC governance functions. The post holder will also represent the service and report to the ACW ICSUs, ensuring that Wood Green CDC operates within a comprehensive governance framework that continuously monitors and enhances the quality of care across the following key areas:

Imaging (X-ray, CT and MRI)

Ophthalmology services

Pathology (Phlebotomy)

Main duties of the job

MAIN DUTIES

3. Governance and Quality

3.1. Lead the WG CDC Monthly Quality Governance meeting and follow up on actions and escalations to achieve the required resolution and compliance.

3.2. To support completion of the performance indicator dashboard and individual quality and accreditation reports for WG CDC to the ACW ICSU quality and safety group.

3.3. To link with the WG CDC Governance meeting and ICSU quality and safety meetings ensuring that information and learning is shared across the various disciplines e.g. Health & Safety, PALS, Complaints as well as Audit and Clinical Effectiveness

3.4. To support the WG CDC Leadership in the ICSU Quality & Safety group meeting to review and analyse internal patient safety data and intelligence identifying themes and trends as directed that produce change and learning reducing clinical risk.

3.5. Support the Accreditation and Quality manager in the maintains of quality management system (Q-Pulse) for compliance standard

3.6. In conjunction with the Accreditation and Quality Manager, develop and produce comprehensive reports for WG CDC on incident safety and audit management. This includes collecting and managing incident reports within the Q-Pulse system, conducting ongoing compliance audits, and analysing incident trends to support continuous improvement.

MAIN DUTIES

1. Incident management and Serious Incident investigations

1.1 Support the WG CDC Management and Leadership team to ensure that all WG CDC adverse incident investigations are appropriately investigated and acted upon to improve patient/staff safety.

1.2 Chair the WG CDC monthly Quality Governance meeting and ensure timely follow-up on actions and escalations to drive resolution and maintain compliance.

1.3 Design and deliver training, in collaboration with the Quality and Safety Group, on incident and risk management for dissemination across the CDC and ACW ICSU.

1.4 Support the WG CDC leadership in developing a no-blame culture that promotes learning and continuous improvement.

1.5 In collaboration with the WG CDC leadership team, develop, review, and update Standard Operating Procedures (SOPs) in line with Trust governance processes and requirements.

1.6 Support quality, risk and service lead within the CDC and ICSU in the investigation of serious incidents via Root Cause Analysis (RCA).

1.7 Work with the Imaging Governance and ACW quality and safety Leads to ensure action plans arising from serious incidents are completed within agreed timescales and that evidence is submitted to the relevant committees.

1.8 Monitor compliance with the Duty of Candour in respect of patient safety incidents, providing support for clinical staff and ensuring that the duties of the Trust are met.

1.9 In collaboration with the Quality and Safety group, promote a culture where staff are confident in reporting adverse incidents and investigating them using root cause analysis.

1.10 Ensure delivery of the rolling programme of governance activities in line with trust, division and CDC service requirements.

1.11 Liaise with the ACW ICSU Quality and Safety Group and Patient Experience Team to agree and achieve deliverables, as appropriate

1.12 Manage, monitor, and review patient and staff surveys, producing a monthly report to inform service improvement.
