We are looking for someone who has excellent holistic personalised care skills, able to support clinicians to improve the physical & mental health and wellbeing of clients living in the borough of Haringey. Although this is advertised as Band 6 post, we are happy to accept applications from Band 5 in a development post to a band 6 dependent on achieving Band 6 competencies during the probation period.
The role is a great opportunity to work with other professionals in the team including Physiotherapists, Social Workers, Pharmacists, Occupational Therapist, Community Matrons, Mental Health Workers and Care & Dementia Navigators, supported by a fantastic administration and MDT coordinator team.
You will be committed to addressing Health and Social Inequalities for your clients and work towards the team purpose of; Managing health, wellbeing and preventing crises through innovative partnership working.
You will be supported to develop skills and competences and have the opportunity to stretch and develop your skills with the support of the team and with regular professional supervision.

Main duties of the job
Job Summary
To provide high quality efficient and effective holistic occupational therapy assessment and intervention to the clients care coordinated within the MACCT MDT.
To provide specialist occupational therapy assessments and creative personalised treatment programmes co-produced alongside client's strengths and their support network to support them to manage with complex health conditions both physical and psychological and to support the stabilisation of their social situations.
To promote good collaborative working links with partnership agencies across the health, social and voluntary sectors to ensure the clients have the best possible support network to prevent unplanned hospital admissions.
To be an active motivated member of the multidisciplinary team and participate in projects and service developments as identified by the successful candidates, their colleagues and managers including audits, quality improvement projects and the promotion of integrated personalised care approach. To provide support and clinical supervision to colleagues or junior staff, and to share specialist knowledge with others working within the MACC Team and the associated teams, hubs, care providers, clients and their support networks.
