Junior Clinical Fellow in Acute Medicine

Job summary

This is an exciting opportunity for a Junior Clinical Fellow who has completed Foundation training (or the equivalent) to work with the Acute Medicine team at the front door of the hospital. You are looking for additional experience in Acute Medicine to develop your knowledge and skills in managing acutely unwell patients and providing excellent clinical care. You may be exploring a career in a medical speciality, considering applying for Internal Medicine training or looking to gain additional experience in General internal Medicine to support a career in another area.

You will work a Monday - Friday 8am - 5pm standard day work schedule with the option to do some ad hoc on calls if you wish (to be discussed at interview). This post is suitable for someone who wishes to work a stable standard working week while developing their clinical skills in the Acute Medicine setting.

You will predominantly be working with the 'post take' and medical outliers team who focus on care of patients in the first 24 hours of their arrival in hospital. There is a dedicated consultant ward round daily and you will be primarily responsible for the cohort of patients supported by the consultant and the rest of the Acute Medicine team. You will have the opportunity to learn procedural skills including using Ultrasound to guide procedures, and develop your clinical skills and knowledge across the Internal Medicine curriculum.

Job responsibilities

Post take/Outliers responsibilities

You will predominantly be working with the post take/ Acute Medicine outliers team who are responsible for patients in their first 24 hours of their hospital journey.

You will be responsible (alongside other resident doctors) for clinical management and treatment including managing acutely unwell patients and discharging patients. You will be supervised at all times by an Acute Medicine consultant dedicated to this cohort of patients.

You will have a strong skillset in communication both interprofessional and with patients and their families.

You will have a good understanding of patient safety and clinical risk and escalate any concerns in an appropriate manner.

You will be well organized in your clinical work and handover and be able to prioritise effectively.

You will be able to recognize the deteriorating patient and initiate emergency management, and escalate effectively. You will be enthusiastic about learning new procedures and skills and be able to manage uncertainty with confidence.

You will work as part of the wider Acute Medicine team who comprise of 10 consultants and 25-30 resident doctors who work across several clinical areas: Acute Medicine wards, Same day emergency care (SDEC), the Acute Medicine /GIM take and post take. Your work will be mainly focused on the post take cohort but you may have the opportunity to work in the other areas depending on your training interests and career focus.

Acute Medicine ward (AAU)

This is a 34-bed unit based across two adjacent wards (Mary Seacole North and Mary Seacole South). This is staffed on a daily basis by 2 Consultants, a registrar, 2-4 SHOs and 2-3 FY1s with daily Consultant led ward rounds.

Wider AMU staffing includes a dedicated and highly trained nursing, physiotherapy, occupational therapy and MDT team, a frailty team, pharmacy and a full complement of support staff.

There are full cardiac monitoring facilities and isolation facilities, and patients may receive non-invasive ventilatory support on the AAU as well as chest drains, management of level 0 and level 1 patients and a close working relationship with the intensive care, cardiology, respiratory and gastroenterology teams who all in reach to AAU regularly.

Ambulatory Emergency Care (AEC)

The award-winning Ambulatory Emergency Care Unit is staffed by a dedicated team of senior and junior doctors and Advanced Clinical Practitioners who see acutely unwell patients, in an outpatient same day emergency care (SDEC) setting, providing an alternative to admission for many patients.

The ambulatory care unit has a dedicated Consultant and Senior Nurse who supervise the diagnosis, management and treatment of patients on the unit and provide teaching and support for the many procedures (chest drains, ascitic drains, lumbar punctures, infusions and diagnostic imaging) which take place there.

Acute Medical Take

This is staffed by 1 SpR, 2 SHOs and 1 FY1. There is an average intake of 30 patients over 24 hours. During the day, there is a continuous consultant post take ward round providing the opportunity for immediate feedback and learning. Overnight, Consultant advice is available by telephone, and patients are reviewed on the morning post take ward round.
