Stratified Urology Pathway Administrator

Job summary

We are seeking someone with administrative experience who can balance competing demands and is dedicated to delivering an excellent patient experience. As part of the Cancer Services Team, the post holder will be required to engage with and work alongside health professional and administrator staff working across the patient journey, in including primary, secondary and palliative care. Coordinate the administrative journey of patients on stratified follow-up pathways.

Coordinate the work of the specialist/ teams to ensure there is effective cooperation between Departments and Divisions involved in the delivery of the patient pathway.

Facilitate service improvement to seamline the patient journey and improve the quality of patient care.

Develop information systems, in line with Trust strategy, for the recording and reporting of clinical and performance data.

Actively monitor waiting times for individual patients to ensure targets are not breached.

Main duties of the job

Coordinate the administrative pathway of patients providing a link across Specialties and Divisions. Develop systems to ensure rapid communication and effective co-ordination between all members of the multidisciplinary team (MDT). Deal with non-clinical queries from patients, their careers, General Practitioners and members of the multidisciplinary team.

Service Improvement

Facilitate the Trust-wide modernisation of services in partnership with the MDT to improve the speed of patient access to services.

Record, analyse and interpret capacity and demand data and work with MDT to explore solutions where required. Support clinical teams to improve patient and carer experience. Adopt a leading role in implementing, monitoring and auditing the standards set out. Support clinical teams to undertake internal audit and patient satisfaction surveys. Promote awareness of service developments to all stakeholders. Provide regular reports on service development to MDT.

Team Coordination

Provide administrative support. Liaise with clinical and administrative staff across Divisions. Implement systems to ensure that the information necessary for effective team functioning is available.

Ensure actions plans are produced and recorded for each patient and communicate this information to the relevant GP within 24 hours. Be responsible for and ensure that any appropriate follow-up actions are carried out. This may involve contacting patients.
