Job summary

JOB SUMMARY

To manage the Generalist Community Children's Nursing (CCN) Team, which includes supporting hospital @home nurses and working key CNS's within the community and administrators. The service is provided to children and young people living in the London borough of Islington. The main responsibilities for the post are to:-

o To ensure that the nursing needs of children are identified, with appropriate strategies, to meet those needs at home or in other community settings. Children may require short term acute interventions and clinical care through to case management for children with long term conditions, complex health needs and physical disability.

o To undertake clinical activities and responsibilities for an appropriately reduced caseload and at times act as the multi-agency lead professional.

o To be accountable for Generalist CCN team standards of practice according to Trust and National guidelines.

Main duties of the job

1 Main Duties include: managing the children's community nursing team.

s Managing the caseload and supporting the team to undertake visits, this includes children from the active and dormant caseload.

Ensuring monthly caseload reviews

Attending DPM with staff.

Holding a small caseload and supporting young people through transition.

Working closely with MDT and other AHP.

To provide regular caseload management supervision including child protection advice to members and facilitate a rolling programme of caseload reviews.

To ensure robust standards and frameworks are in place, in line with current recommendations, to ensure children always receive high quality family centred care.

To arrange quarterly safeguarding supervision with the trusts named safeguarding lead.
