Job summary

A fantastic and exciting opportunity has arisen within Whittington Health NHS Trust for an experienced and highly motivated Band 8a Clinical CHC Team Lead.

The post will cover all aspects of Continuing Healthcare provision in Haringey and Islington consisting of patients in the community and provide clinical and nursing leadership and oversight of operational management for each area. The post provides an invaluable opportunity for a nurse to experience working across community services and makes this an ideal opportunity for applicants who are looking to further their clinical, service development and leadership skills in the NHS.

The post is full-time, 37.5 hours per week Monday - Friday 9-5pm. We also have flexible working policies to support individuals achieve the work - life balance that their situation might require.

  • The post holder will manage a team of staff in Haringey and Islington boroughs providing excellent management and professional leadership to the Continuing Healthcare team.
  • The post holder will be responsible for ensuring the statutory requirements as laid out in the National Framework for NHS Continuing Healthcare (CHC) and NHS - Funded Nursing Care (FNC) are implemented.
  • The post holder will develop robust processes and procedures and policies which are compliant with legislative and national guidance and in accordance with ICB responsibilities.

Main duties of the job

  • To be responsible for ensuring that all CHC processes implemented are compliant with legislative and national guidance and in accordance with the responsibilities of the ICBs.
  • To work with Mental Health Trust and LA partners for Section 117 of the Mental Health Act in relation to after care, in the context of continuing care.
  • To maintain a robust decision making process including for the ratification of MDT recommendation and where there is a panel, to monitor its running and processes are in accordance to the CHC Framework for Older People, Physical Disabilities & Complex Needs, Mental Health and Learning Disabilities.
  • To manage contentious situations in relation to disputes between partner agencies or withindividual patients and relatives.
  • To monitor the quality of care packages for continuing healthcare and funded nursing care through reviews and case management.
  • To have lead responsibility for establishing and coordinating arrangements across a range of agencies including NHS providers, third sector and independent sector providers.

  • To hold lead responsibility for delivery of the action plans relating to continuing healthcare, including leading service reviews, developing business cases and procuring services to meet identified gaps.

  • Responsible for the management of all staff within the Islington and Haringey CHC team.
