Job summary

The post holder will be responsible for supporting the Continuing Care team, maintaining clear systems to support the administrative duties required to maintain the smooth running of the team. An essential component of this post is excellent communication skills in order to develop the trust and commitment of the team.

Main duties of the job

The role focuses on ensuring the team operates effectively by providing exceptional project, administrative, and analytical support to the CHC team. Key duties include coordinating CHC assessments and reviews, handling internal and external queries, maintaining compliance with NHS Framework guidelines, and overseeing stakeholder communication. The post holder will also assist in preparing reports, managing inboxes, organise meetings, take minutes, and manage client records. Proficiency in relevant IT systems and adaptability to changing priorities are essential. This role requires a meticulous approach to data management and communication while upholding confidentiality and supporting the team's operational excellence.
