Job summary
An exciting opportunity has arisen for a motivated junior doctor to work as Frailty Fellow FY3 in a multidisciplinary acute frailty team within the Whittington Health Integrated Care Trust.
The post is ideal for candidates who would like to take a step off the training ladder (as a FY3) or for trainees returning to training or clinical practice. The appointee will gain excellent experience of interface geriatrics between primary and secondary care and will suit candidates who are considering a career in primary care, geriatric medicine or acute medicine. The Whittington has an excellent reputation for career advice and education and a good track record in preparing candidates to successfully apply for specialist training.
We are looking for a flexible and dynamic individual with a comprehensive clinical background, excellent interpersonal and communication skills, and the ability to work independently. Experience in delivering training is desirable and background experience of working clinically with older patients living with frailty and having completed FY2 programme with at least 4 months of previous experience in either Emergency Medicine, General Practice or Acute Medicine is essential.
Main duties of the job
The successful candidate will work to support the Acute Frailty Service which works to avoid unnecessary admissions to hospital for older people and support patients to be more independent in the community for as long as possible.
The post holder will work as part of the multidisciplinary Acute Frailty Team within the Emergency Department (ED) and Ambulatory emergency Care (AEC)/ Same Day Emergency Care (SDEC) as well as the Acute admission wards under the direct clinical supervision of the Frailty consultant geriatrician.
The successful post holder will be able to take a lead in the comprehensive geriatric assessment of patients referred to the Frailty Team. They will autonomously work as part of a multidisciplinary team providing patient-centred clinical care, encompassing the skills of assessment, examination, diagnosis and treatment of patients. The post is intended to provide improved outcomes for patients by improving timeliness and consistency of care for older patients with frailty seeking emergency care and referred to the Acute Frailty Pathway, promoting same day discharge and community management of older patients with frailty, whenever appropriate.
There is no on-call requirement for this post, although expressions of interest to perform on-call duties will be welcomed.
The post will be offered for a 12-month contract, but flexibility will be applied for less than full-time working or for a reduced contract period.

The successful applicant will work as part of the frailty MDT for patients on the frailty pathway, in ED,ambulatory care and the acute medical wards contributing to their Comprehensive Geriatric Assessment, therapy input and Consultant Geriatrician support. This will include the initial assessment of patients, organising relevant investigations in an outpatient setting and arranging a supportive discharge including liaising with the virtual ward.Depending on the development of the service the role may develop during the year to become more integrated with opportunities to join the enhanced virtual ward and community teams. The successful applicant can expect to develop skills in the assessment and care of older people in particular common presentations such as falls, delirium, polypharmacy and decreased mobility.
The post holder will ensure evidence-based practice is inherent throughout all aspects of care delivery and treatment and will ensure this knowledge is disseminated via robust plans.

There are regular weekly and monthly meetings held in the hospitals Postgraduate Centre. Programmes are regularly circulated. In addition there is weekly COOP departmental teaching and insitu simulation training which the successful applicant can be involved with.
The Ambulatory Emergency Care Consultants are committed to the supervision and development of this post. All post holders will have a Consultant allocated as their Educational Supervisor for the duration of the post.
- To screen for patients in the Emergency and Acute wards who meet the criteria to be seen by the frailty service and to proactively identify patients for the service.
- To see and treat patients as they present to the department on the frailty pathway and commence comprehensive geriatric assessment/frailty reviews.
- To liaise with the rest of the MDT to complete the assessment and plan safe discharge
- To liaise with the community teams and GPs to maintain and ensure continuity of care and appropriate follow-up of patients
- To liaise with appropriate specialty teams where necessary to optimize care for patients.
- To liaise with the imaging department to arrange urgent diagnostics.
- To actively monitor and help manage patients queues in conjunction with the senior nurse and HCA
- To undertake, in conjunction with the Consultants, daily review of investigation results returning to the department with the arrangement of patient follow up, when necessary.
- To work closely with the Emergency Department, Urgent Care Centre and Medical Assessment Units and the COOP Department
- To participate in the weekly Multi-Disciplinary Team meetings
