Job summary
We are looking for an enthusiastic motivated Transformation Clinical Nurse Specialist (CNS). For Virtual Wards, North Central London (NCL)who has a passion for education. Who is seeking a to develop a role that is varied, interesting and crucial to ensuring excellent patient experience every step of the way.
Virtual wards allow patients to get the care they need at home safely and conveniently, rather than being in hospital. They care for patients who would otherwise be in an inpatient bed. In a virtual ward, support can include remote monitoring using apps, technology platforms, wearables. CNS is a 12-month leadership and development role.
The post holder(s) will work alongside an antimicrobial stewardship (AMS) pharmacist and the wider OPAT team implementing novel methods of improving and designing the pathways for virtual ward patients who are receiving antibiotic medication, with a focus on avoiding admission and reducing length of stay for inpatients receiving intravenous antibiotics by transitioning them to a virtual ward pathway
Main duties of the job
The band 7 virtual ward transformation CNS for advanced IV skills will be competent to work in an OPAT service. It is envisaged that the band 7 transformation CNS will work autonomously with the support of the AMS pharmacist and OPAT clinical lead, develop and ensure training of nursing staff across all NCL virtual wards (and other community nursing teams if time permits), education of patients and OPAT service development and research.
