Job summary
The post holder will be driving an innovative service which engages children's nurses to work, run and deliver clinic's within GP practices. The service is provided to children and young people aged 1 - 18 years, living in the London Borough of Islington who present with viral induced wheeze, asthma, eczema, constipation, and nocturnal enuresis. The service aims to minimise attendance, admissions, and re-admissions to hospital, for children with these conditions.
Main duties of the job
The band 7 nurse will support the community matron in embedding the service across Islington and will engage in reviewing data and developing the service.
To develop an integrated team approach with Practice Nurses where possible to develop their skills to providing more care to the children within their practice.
To support the daily operational management of the service and provide leadership and clinical support to band 6 Children's Nurses and Practice Nurses across Islington.
Work to review those children who are frequent flyers to Emergency Department, liaising with E/D staff.

To meet regularly with clinical leads and continue the strategic development of the service within a structured framework. To foster and build effective working relationships within the service; childrens nurse specialists, the community childrens nurses, and across multi-agencies including E.D. staff and general practices.
To ensure that the nursing needs of children are identified with appropriate, strategies to meet the needs within community/primary care settings.
To support the implementation of local paediatric pathways in relation to, asthma, constipation, eczema, viral wheeze including nocturnal enuresis. working to reflect local and national guidelines to improve the outcome of children.
To train health professionals in the management of long-term conditions when requested thus improving the confidence and competence level of practice staff.
To contribute to the MDT approach to reviewing frequent flyers to ED with the named consultant/ teams.
To lead the paediatric primary care standards of practice according to Trust and National guidelines in accordance with NMC Code of Conduct.
To work to reduce referrals to the 10-12 clinic at Whittington Hospital and the urgent care clinic at UCLH where possible.
To monitor, review paediatric primary care data with the matron and the team and implement service improvement where possible, including reviewing DNA data.
To ensure admin team have a clear understanding of the booking process and be involved in attending data meetings were possible.
To accurately record and save data on relevant systems to lead service improvement.
To ensure the service obtains patient feedback in a timely manner
