As an integral member of the management team, the Intermediate Diabetes Specialist Service Lead will support the Haringey and Islington Community Diabetes Service in the delivery of key performance targets. They will be accountable for the provision of effective professional nursing leadership and proactive resource management across the specialist nursing and be expected to create an environment of continuous quality improvement and professional development, ensuring the delivery of patient care is regularly reviewed and that service developments are implemented when necessary
Main duties of the job
The post holder will be an expert practitioner in Specialist Diabetes Nursing. They will work independently and without supervision, to deliver evidence-based care, education and information to people with diabetes and their families referred from Primary Care or from within Whittington Health

Management/ Service Development
Be fully involved in supporting the operational management of the diabetes team.
- To develop and manage the service including line management of junior team members, within finite resources.
- To ensure adequate and safe provision of services that meet patient need statutory requirements and professional responsibilities including challenging and addressing poor performance and practice.
- To be a member of diabetes networks to support and develop diabetes services in Haringey and Islington
- Support the long-term strategy for diabetes care, ensuring involvement from key stakeholders, commissioners, and experts in diabetes to steer, support and validate the work in this area.
- Report and account for activity through Key Performance Indicators.
- Ensure that the team is providing optimum productivity.
- Responsible for reviewing the staffing resources within the team and recruiting to any vacancies as they develop.

Clinical
- The post holder will be expected to provide 50% of their time to clinical delivery of care.
- Demonstrate a high level of advanced clinical competence in diabetes management; based on in depth knowledge, research and evidence based findings
- Plan and manage nurse-led intermediate diabetes clinics in partnership with key Allied Health Professionals, working in conjunction with secondary and community services for people with complex needs and uncontrolled diabetes. Initiation and management of triple therapy including insulin and non-insulin injection treatment.
- To ensure the implementation of national, local and Trust standards of best practice and quality and monitor performance against these that informs the corporate performance review framework.
- Demonstrate autonomous clinical practice through direct referral, comprehensive client centred assessments, initiate therapeutic regimes e.g. insulin, specific interventions and evaluation of practice using approved protocols.
- Ensure that all Specialist Nurses participate in compliance audits and action plans are implemented and regularly reviewed.
- Ensure that Diabetes Service have systems in place to identify report and manage risk which involves taking action and monitoring work areas in accordance with Health and Safety legislation
- To undertake formal investigations reporting back within the agreed time limits and ensuring lessons are learnt, action plans are implemented, and progress is monitored and reported appropriately.
- To ensure that mandatory audits are completed and actioned and their teams in an appropriate and timely manner
- Be responsible and accountable as a Named Nurse for a caseload of new and complex patients for a limited time, working in tandem with colleagues in primary care, ensuring their skills are developed to a point where such patients can be safely discharged to primary care.
- Provide evidence based clinical advice on diabetes management to people with diabetes, their carers and health care professionals.
- To work to promote good practice in diabetes care across both boroughs by offering leadership, guidance, education and mentorship to primary health care professionals.
- Support people with diabetes and their carers to achieve individualised goals and care planning to maintain their physical and psychological well being, encouraging self management.

Educational
- Contribute to the formulation of education strategies to ensure staff within the specialist areas are competent in their roles and practice.
- Identify educational requirements for patients, carers and professionals and develop education delivery strategies that meet the patients needs.
- Participate in group education programmes when required for people with diabetes at different times in their disease progression i.e. newly diagnosed Type 2, insulin initiation and ongoing care, to ensure that both physical and psychological aspects of diabetes self-management are addressed.
- Support the Diabetes Self Management Programme (DSMP) delivered by the Whittington Self management team and if required act as DSMP lead post training.
- Maintain own knowledge and expertise by attending conferences, workshops and study days.
