Screening Pathway Navigator (Bowel Cancer)

Job summary

The Screening Pathway Navigator will form an integral part of the multidisciplinary Bowel Cancer Screening Programme (BCSP) team in caring for the physical, social and psychological needs of patients and carers following a FIT+ referral, throughout the diagnostic pathway and up to the point of referral for cancer or discharge to recall or surveillance.

The post holder will liaise with the Specialist Screening Practitioner (SSP) team, who will have regular direct contact with patients, providing support, advice, signposting and assistance for those requiring a diagnostic test (Screening Colonoscopy or alternative test) following an abnormal (FIT positive) screening test. The role will also involve administrative tasks deemed suitable to the role by the Lead Nurse and Programme Manager.

Main duties of the job

  • Triaging telephone calls and providing basic advice and support.
  • Liaising and referring to other appropriate healthcare professionals.
  • To assist the SSP's to ensure that patients are signposted in a timely manner to appropriate services following their electronic Health Needs Assessment.
  • To ensure the appropriate management of patient information and the co-ordination of patient pathways through collaborative working with medical secretaries, MDT coordinators, clinical staff and other departments as required within the Trust/region
  • Assist in the development of care pathways and implement improvements to the administrative element of the pathway
  • Ensure that directorate administrative and clerical functions are carried out to the highest quality standards by monitoring and suggesting improvements to services.
  • To lead on the health promotion of the service.

  • The role will require highlighting any potential delays in the patient pathway and working to mitigate the impact of these problems to the patients. The post holder will be required to work with the senior staff to support and ensure that the patient and carer experience is central to every stage of their journey.
  • To support the BCSP SSPs by responding to the noncomplex workload, and referring to the SSPs as appropriate
  • To assist in the timely communication of information to the patients at the end of their diagnostic pathway and consistent with current screening protocols.
  • To ensure the appropriate and timely booking of outpatient appointments.
  • To undertake relevant audit/s to ensure the patient pathway is maintained within the 28 day FDS, highlighting where this is not possible and why, and expediting interventions to reduce the length of the pathway for individuals affected, where this is possible to do so.
  • To assist in maintaining the BCSPs patient database (Bowel Cancer Screening System), including monitoring of all system alerts to ensure all actions are completed by the relevant team/clinician and there is no avoidable slippage in the patients pathway
