Job summary
The post holder will work as a member of the Trust Practice Development Team, based within the Community and Integrated Services Group, working in partnership with identified Coventry based Nursing Homes and other stakeholders such as Clinical Team Leaders, Service Leads, Clinical Educators, the Integrated Care Home Team and others.
A full UK driving license is essential for this post. Please note that an international driving license is not accepted.
Secondment will be considered for internal candidates - If you are successful after interview, you must obtain approval from your current line manager to accept the offer.
Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
Main duties of the job
The post holder will support the implementation of evidence-based practice and the professional development of clinical staff working in Coventry nursing home settings by reviewing care pathways, developing and delivering education programmes and reviewing data around avoidable hospital attendance.
Through delivery of the objectives above, the post holder will be expected to contribute to the reduction of avoidable hospital attendances for Nursing Home residents, contribute to a reduction in the number of 'Notices of Concern', impact on the number of patient safety incidents reported through nationally supported reporting systems, increase pre-registration student nursing placement capacity and the readiness of the Nursing Homes to support learners, and increase workforce confidence, competence and perception of support.
The post holder will be clinically based, placing the clinical environment at the centre of their activities, and will participate in specific projects related to nursing practice.
