Job summary
Coventry Intergrated Palliative Care Team supports patients, families and carers with specialist palliative care needs, palliative support, and end of life care within the community setting. The post holder will be required to work as part of the integrated night palliative care multidisciplinary team and will work with other health care professionals to provide specialist advice, support, and high-quality patient centred care to those affected by malignant and non-malignant disease at the end of life and generalist community patients that require urgent interventions overnight within a 2-hour rapid response. This will be provided 7 days a week 19:30-08:00 on a shift rota basis.
Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

Main duties of the job

- Triage and accept referrals direct from other disciplines, patients and carers and refer to other agencies where appropriate.

- Contribute to the development and review of policies, procedures and protocols within the Trust related to palliative and generalised care.
- Actively manage unscheduled visits to patients in a 2-hour rapid response timely manner.
- Undertake comprehensive risk assessments associated with the care of patients to ensure nurses and carers safety.
- To safely refer patients on to the relevant health care provider as and when appropriate.
- To manage the palliative care phone out of hours.
- To escalate any concerns or support needed to the appropriate MDT member.
- To competently perform a range of clinical skills related to the post.
- Ability to work without direct supervision and to solve problems.
- Work on a rota basis to meet the service requirements.
- Act as a role model for other staff and students demonstrating high standards of practice and professional conduct.
- Maintain a professional image.
- To undertake continuous evidence-based learning required for the position.
