Dementia & Delirium Clinical Nurse Specialist

Job summary

The role provides specialist support to the Clinical Nurse Specialist team in the care of patients with dementia and delirium. Acting as an expert resource, the Dementia CNS delivers assessment, clinical advice, education, and training to embed best practice across the organisation. The post holder leads service development and quality improvement initiatives, supports compliance with national standards, and advocates for patients and carers to ensure dignified, person-centred care.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

Main duties of the job

  • Work collaboratively with multidisciplinary teams and other professionals to assess, treat, and improve the identification and management of co-morbidities and frailty in people with dementia

  • Deliver holistic interventions to promote health and wellbeing for people with dementia and their carers/families

  • Develop, implement, and support person-centred care plans, including positive risk management

  • Support people with dementia, carers/families, and staff to understand and respond to changes in behaviour, cognition, and relationships

  • Actively engage people with dementia and cognitive impairment, and their carers, in all stages of intervention, ensuring their views are heard and respected

  • Promote and support advance care planning, including best-interest decision-making, in partnership with colleagues and families

  • Liaise closely with social workers and community mental health services to support timely interventions and safe discharge planning

  • Maintain accurate, up-to-date clinical records in line with local policy and national GDPR requirements

  • Provide expert clinical care and strategic leadership for patients with dementia and delirium in line with the UHCW Dementia Strategy

  • Monitor key performance indicators and ensure compliance with national dementia standards

  • Lead and support staff education and engage with carers

  • Use data and audit to drive quality improvement, enhancing patient safety and experience
