Critical Care Outreach Practitioner

Job summary
The practitioner will work as part of the critical care outreach service. They will provide expert clinical practice, care, and intervention for the acutely ill, deteriorating and critically unwell ward patient across the trust on a 24-hour shift rotation that covers 365 days per year.
The practitioner will perform patient assessments, undertake appropriate investigations and initiate, evaluate, and modify a range of interventions to prevent patient deterioration. They will assist and facilitate the timely admission and discharge of patients to and from the critical care area by early care intervention and supportive of their continuing recovery within the ward environment.
Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
Main duties of the job
- Support staff, patients and family during difficult situations arising in the clinical area, e.g. breaking bad news or following an unexpected event.
- Provide mentorship and clinical supervision for healthcare students and junior healthcare staff.
- Assist with education and supporting patients with a tracheostomy. Assist with weaning of tracheostomies.
- Support education and training provision of identified staff groups in relation to the deteriorating patient management.
- Use the SBAR tool to communicate clinical concerns.
- Deliver care according to local and national guidelines to ensure delivery of evidence-based care.
- Participate in the maintenance of quality governance systems and processes across the trust.
- Participate in ongoing evaluation of the outreach service and support implementation of change as appropriate.
- Remains up to date with clinical and professional developments.
