Complex Flow Liaison Team Leader

Job summary

  • To work autonomously within the hospital demonstrating effective time management skills to manage a varied caseload and complexity.
  • To assess, advise and support the ward with the appropriate Discharge Pathway to meet the individual patient's level of need. For complex pathway 0 patients including Domestic violence, modern day slavery, human trafficking and homelessness.
  • To contribute to effective discharges, ensuring adherence to Trust policy at all times.
  • To support and upskill the wards and MDT on discharge planning and pathways and appropriate service pathways.
  • To participate in the planning, development and evaluation of the Patient Flow Liaison team, holding responsibility for team development.

Main duties of the job

  1. To lead and provide support to the Patient Flow Liaison Team, with an overview of the teams' caseload, to ensure the correct level of support is available for the team.
  2. To deliver training in board rounds, Pathways, and system support tools including Discharge checklist to support assessments as required ensuring safe discharge planning with the correct identified level of support for discharge.
  3. To support discharge for those patient's case managed by IDT when IDT not available.
  4. To support increase in the percentage of pathway 0 patients.
  5. To oversee assessments and appropriate referrals for discharge planning.
  6. To communicate with patients / families / carers / care Homes of the discharge process and plans and lead on discharge planning meetings as required to support the Flow Liaison team.

  • To work independently on ward areas, seeking senior level of support as required on tasks outside of identified competency.
  • Provide education, support and advise to the Flow Liaison team and ward staff on the discharge process and pathways
