iSPA Clinical Triage Nurse

Job summary

An exciting opportunity has arisen for a skilled and compassionate Band 6 Clinical Triage Nurse to join our Community Services team. This role is pivotal in ensuring patients receive timely access to the most appropriate care pathways through effective clinical triage and coordination.

The post holder will be responsible for assessing referrals, prioritising clinical need, and liaising with multidisciplinary teams across community health services. You will support safe and efficient patient flow, enabling early intervention and reducing unnecessary hospital admissions.

This role requires excellent clinical judgement, communication skills, and the ability to work both independently and collaboratively within a fast-paced environment. You will be part of a supportive team committed to delivering high-quality, person-centred care in the community.

Our working hours are between 0800-2000 7 days a week and you will be required to work shifts within those times and days.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

Main duties of the job

  • Triage referrals into community services using evidence-based clinical protocols.
  • Identify urgent care needs and escalate appropriately to ensure patient safety.
  • Liaise with GPs, hospital teams, social care, and other community providers to coordinate care.
  • Provide clinical advice and support to patients, carers, and professionals.
  • Contribute to service development, audit, and quality improvement initiatives.

  • Triage and accept referrals direct from all referral sources into the Community Services and sign posting on to other agencies where appropriate.
  • Undertake comprehensive risk assessments associated with the care of patients to ensure nurses and carers safety.
  • Act as a role model for other staff and students demonstrating high standards of practice and professional conduct.
  • Support the Senior Management Team in ensuring all obligations of service delivery are met.
  • To clearly identify urgent and non-urgent referrals and allocate patients to the relevant pathway of care.
  • To utilise the Trusts and other approved electronic clinical systems to access patient information to support the decision-making process.
  • Undertake comprehensive holistic telephone or other virtual technology assessment of the patients presenting problem.
  • With consent, liaise with relevant healthcare or other professionals, relatives and carers to acquire necessary clinical information to inform comprehensive triage of referral.
  • Ensure timely escalation of present time demand and capacity issues in the team daily to clinical and operational leads.
  • Allocate referrals requiring an urgent (timed) response to the rapid response team to ensure timely patient care.
  • Ensure practice is evidence based through undertaking audit and critically appraising literature to achieve a high standard of care.
  • Involve patients and carers in planning and delivery of care.
  • To competently perform a range of clinical triage skills related to the post.
  • Ability to work without direct supervision and to solve problems.
  • Work on a rota basis to meet the service requirements.
  • Maintain a professional image.
  • Attend professional meetings and case conferences as deemed necessary.
