If so this could be the perfect opportunity for you! Experience working as a PCN Dietitian is desirable but not essential, provided you have substantial experience in a specialist dietetic role.
The RoleAs a PCN Dietitian, you will play a key role in proactive, patient-centred dietary care in primary care. You'll work with GPs, nurses, pharmacists, social prescribers, health coaches, and other allied health professionals to provide evidence-based nutritional interventions that help patients manage and prevent long-term conditions.
Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
Main duties of the job
- To carry out dietetic assessment appropriate to role and experience, provide nutritional diagnosis and provide a dietetic treatment plan for patients / clients.
- To hold responsibility for own patient caseload including patients with complex presentations
- Maintain clinical records in line with local / Trust documentation guidelines
- To liaise with dietetic colleagues and other members of the multidisciplinary team as necessary e.g. Medical, nursing, catering and other health care professionals
- To undertake leadership and line management duties as required for the post
- To work within National and Trust policies / guidelines; and HCPC and BDA standards and codes of conduct
- To have an in depth understanding of national and local standards and able to monitor own practice against these
- To participate in bank holiday working and work flexible hours to meet the needs of the patients if required

Job responsibilities
- To undertake complex dietetic / nutritional assessments (including interpretation of biochemistry, anthropometrics, clinical condition and diet history) to determine nutritional diagnosis
- Use expert clinical reasoning skills to provide highly specialist individual dietetic advice and treatment plans for patients who may present with multiple / highly complex clinical conditions taking into consideration patients own goals and incorporate discharge planning as appropriate
- Evaluate patient progress, reassess and alter treatment plans as required
- Communicate complex nutrition and dietetic information in an understandable form to staff, clients and carers using teaching, motivation and enhanced counselling skills including situations where there are barriers to change
- Take a lead on department audit and service development work (and research if appropriate to role), translating findings into practice
- Plan and organise the clinical supervision of student dietitians on clinical placement including planning, timetables and written assessments
- Act as an expert advisor and educator to other healthcare professionals and other Dietitians in own specialist clinical area of work
- Line management, operational lead and service lead responsibilities if / as required
- Engage in department and own CPD activities and lead on supporting the learning and development of others, particularly more junior members of the team
- Cover the work of colleagues and, if required, deputise for senior / management colleagues during periods of absence or peak workloads
- Participate in health promotion activities of the department
- Participate in other duties as required
- If appropriate to role / speciality (i.e. if identified as a service need / change in service or highlighted on appointment) Prescribe medications within agreed scope of practice as a Non- Medical prescriber in line with Trust policy, clinical guidelines and evidence-based care
