If so, then we are keen to hear from you. We are looking to expand the leadership team within our Willenhall Locality and welcome you to apply for the position of Community Nurse Sister. You will be supporting the staff within the team, have the privilege of delivering high quality nursing care to our patients and supporting those around them to ensure we keep them in their own homes for longer managing either their long or short term conditions.

Car driver/owner is essential. We offer a variety of shift patterns to support flexible working and maintain a service provision 08:00 - 20:00, 7 days a week.

Main duties of the job

Responsible for the coordination and provision of planned and unplanned care to patients and their carers in a defined geographical area working as part of a 24 hour nursing service.? Triage and accept referrals direct from other disciplines, patients and carers and refer to other agencies where appropriate.? Case manage packages of care and work within the framework of the community nursing team by providing holistic care? Undertake comprehensive risk assessments associated with the care of patients in order to ensure nurses' and carers' safety.? Act as a role model for other staff and students demonstrating high standards of practice and professional conduct.
