Job summary
We have an exciting opportunity for an enthusiastic and motivated dietetic assistant practitioner to join our trust multi-disciplinary nutrition team to support patients with intestinal failure and complex gastrointestinal disorders in this full time (37.5hrs), 12 month fixed term, band 4 role.
You will work alongside the nutrition team dietitians to support the assessment, monitoring and follow up of patients with intestinal failure or complex nutrition support needs. You will provide support to patients on their individual dietary plans, take anthropometric measurements and assist in the discharge processes and follow up of patients under the care of the nutrition team. A key part of this role will be liaising with catering and training of ward staff.
You will be joining a dietetic department that already has established dietetic assistant practitioner posts within the nutrition team and other speciality areas who will be able to offer peer support.

Main duties of the job
To work under the supervision of the Nutrition Team Dietitians to provide support with the oral dietary management and nutritional monitoring (documentation, anthropometrics, biochemical) of patients with intestinal failure or complex nutrition support needs in both the inpatient and outpatient setting.
To assist in the discharge planning process for patients going home on enteral or parenteral nutrition.
o To work as part of the multidisciplinary Nutrition Team, under the supervision of the Nutrition Team Dietitians, supporting the provision of high-quality feeding practice and nutritional support at ward level and dietary education to patients
o To work under the supervision of the Nutrition Team Dietitians to provide follow up for patients requiring dietary manipulation, oral nutrition support or enteral feeding, using behavioural management techniques to overcome barriers to change
o To liaise with catering, nursing and dietetic staff to ensure the provision of a appropriate meals for in-patients with intestinal failure or complex gastrointestinal disorders
o To assist in menu planning including carrying out dietary analysis as appropriateo To obtain patients, weight, height, calculate BMI and check Nutrition Risk Score ('MUST') as appropriate

To measure patients anthropometry (i.e. mid upper arm circumference, tricep skinfold thickness, hand grip dynamometry and bed scale weights) in selected patients to monitor nutritional status
To obtain patients biochemical results using the electronic patient record (EPR) as requested by Dietitians using defined criteria
To print out and send biochemistry forms and bottle as requested by the Nutrition Team Dietitians
To review selected patients (inpatient or outpatient telephone follow up) with a care plan in place by Nutrition Team Dietitians to assess progress towards goals
To encourage patients to eat and drink their prescribed nutrition to maximise their nutritional intake and status
To liaise with Nutrition Team Dietitians and ward staff regarding patients monitoring and progress
To review prescriptions of nutritional supplements and snacks within agreed boundaries.
To arrange for TTOs of supplements to be given to patients on discharge as requested by Nutrition Team Dietitians To process communications with GP's to arrange supplements in the community as requested by the Nutrition Team Dietitian To deliver feeding products and equipment to wards for patients being discharged on artificial feeding as requested by Nutrition Team Dietitians To register patients on the internet based database for enteral feed and ancillary deliveries
To input data on inpatient and outpatient activity on the national Intestinal Failure Registry as instructed by Nutrition Team Dietitians To contribute to data input on databases related to nutrition support as instructed by Nutritoin Team Dietitians To analyse food diaries using nutritional analysis computer packages
To develop dietetic displays for in-patients relating to intestinal failure and nutrition support and support to raise the profile of the Dietetic Department as required.
