Job summary
In line with the NHS Long Term plan, the principal aim of this role is to improve the quality of care for patients treated for atrial fibrillation (AF) and atherosclerotic cardiovascular disease (ASCVD) within the Coventry and Warwickshire ICB. The post-holder will be expected to provide a cost-effective clinical pharmacy service, and review patients within primary care who are not optimised for the above conditions.
The post-holder will work with secondary care colleagues, to help identify patients who are not optimised on their lipid lowering therapy in line with local guidance. They will also be tasked with reviewing patients who have a diagnosis of non-valvular atrial fibrillation (NVAF) in primary care and optimising their anticoagulation regimen if appropriate.
The role will involve data collection to demonstrate benefits to patient population in completing these interventions, as well as working closely with other health care professionals to provide teaching and education sessions.
Main duties of the job
- Work as part of the multidisciplinary team (MDT) within the primary care to develop a tailored lipid management plan for identified patients with ASCVD.
- Identification of sub-optimally treated ASCVD patients who are not achieving target lipid levels as outlined in in local guidelines
- Identification of patients with NVAF who require anticoagulation, and initiating therapy where appropriate.
- Review of treatment options for lipid management and anticoagulation in appropriate patient populations, in collaboration with patients
- Counsel and optimise cardiovascular medications in patients where appropriate
- Identification of ASCVD patients, following risk assessment review, of ASCVD patients who are sub-optimally treated for lipid management

- Use clinical expertise to provide evidence-based prescribing advice to ensure effective, appropriate, cost-effective and safe use of medicines and promote a patient-centred medicines optimization agenda for patients with lipid management and co-morbidities.
- Undertake case finding and clinical medication reviews for each patient, utilizing GP computer systems. Discuss findings at the MDT so that the required actions can be built into the care plan.
- Facilitate good prescribing practice within Lipid management across the primary and secondary care interface
- Deal directly with patients/carers as required, to improve concordance with lipid management in order to achieve better health outcomes.
- To provide cross cover for Lipid management clinics with other Senior Pharmacist colleagues.
- Check prescriptions for clarity, safety and efficacy on wards, in dispensaries and the aseptic laboratory and when necessary take accurate drug histories to facilitate this. Query with, and provide the prescriber with appropriate information to effect a change in the prescription when clinically required, to ensure patients safety and effective therapy. In complex situations seeking the advice and support of a senior pharmacist.
- Provide a clinical pharmacy service to patients in the Trust following the departmental and Trust policies and procedures. These will include working to a framework of medicines management and clinical governance including the RPS hospital pharmacy standards.
- Provide information to nursing and medical staff on allocated wards on any aspect of medicines policy ensuring safe practice. Provide information to patients on allocated wards or in dispensaries concerning their medication and answer any resulting questions or queries.
- Provide when necessary, through checking dispensing or supervision of technicians, an efficient medication supply service from any of the pharmacy departments, including the aseptic laboratory.
- To work with the Senior Pharmacist colleagues for the clinical areas to ensure effective medicines optimisation and management. This may include being responsible for audit work, attending meetings and education and training of ward staff. It will also involve ensuring the effective leadership and management of area based teams, deputising for the Senior Pharmacist(s) as needed.
